چکیده

Abstract Background The population of patients with relevant mitral regurgitation (MR) who stand to gain optimal benefit from valve transcatheter edge-to-edge repair (TEER) remains be determined. Prior TEER, a heart-team approach interdisciplinary decision-making is mandatory integrating both the patient profile and co-morbidities. In addition, application established surgical risk scores recommended by current guidelines. Whether alternative prediction more suitable for this fragile cohort burdened various co-morbidities has not been examined in detail. A simplified may achieved using C-Reactive Protein Albumin Ratio (CAR), but its value TEER unclear. Methods This single-center, retrospective study thought determine long-term prognostic accuracy different MR undergoing TEER. For analysis, 316 median follow-up time 5.81 years were included. primary outcome measure was defined as all-cause mortality. ROC analysis conducted identification CAR threshold, subsequently dichotomizing into two groups (CAR ≤0.4 >0.4) estimating their event rate Kaplan-Meier method. we evaluated compared conventional (logistic EuroSCORE Society Thoracic Surgeons [STS] score) C-Index analysis. Results Among high-risk (mean age 75.6±8.2 years, 61.7% male, logistic 19.9% [11.7; 31.6], STS Score 3.8% [2.2; 5.7]), 176 (55.7%) had ≤0.4. Patients an elevated (>0.4) predominantly suffered higher burden co-morbidities, such peripheral artery disease (p=0.001), chronic obstructive pulmonary (p=0.044), kidney (p=0.015). Consequently, these significantly than p=0.002; p<0.001). Endovascular Edge-to-Edge Mitral Valve Repair

Background—The edge-to-edge technique is an accepted method for the surgical repair of a regurgitant mitral valve. This study reports the initial use of an endovascular technology that enables a double-orifice edge-to-edge mitral valve repair without cardiopulmonary bypass in an animal model. Methods and Results—Adult pigs (n 14) were anesthetized, and left thoracotomy was performed for epicard...

Percutaneous edge-to-edge mitral valve repair in high-surgical-risk patients: do we hit the target?

OBJECTIVES This study sought to assess the feasibility and safety of percutaneous edge-to-edge mitral valve (MV) repair in patients with an unacceptably high operative risk. BACKGROUND MV repair for mitral regurgitation (MR) can be accomplished by use of a clip that approximates the free edges of the mitral leaflets. METHODS All patients were declined for surgery because of a high logistic ...

Pulmonary Hemorrhage following Edge-to-Edge Mitral Valve Repair

Mitral valve repair with the MitraClip device has emerged as an effective treatment option for patients with severe mitral regurgitation and contraindications for surgical interventions. While the procedure is not known to cause pulmonary complications, we describe two cases of pulmonary hemorrhage following percutaneous mitral valve repair. The patients did well with supportive care and reinit...

The edge-to-edge technique for mitral valve repair

The edge-to-edge technique was introduced in the surgical armamentarium of mitral valve repair in 1991 and has progressively been used to restore mitral competence in the setting of degenerative, post-endocarditis and functional mitral regurgitation. Appropriate indications and awareness of the important technical aspects of the procedure are prerequisites for a good outcome. The free edges of ...

percutaneous mitral valve repair with the edge-to-edge technique: case series of first iranian experience

mitral regurgitation (mr) is a common valvular lesion in the general population with considerable impact on mortality and morbidity. the mitraclip system (abbot laboratories, abbot park, il, usa) is a novel percutaneous approach for treating mr which involves mechanical edge-to-edge coaptation of the mitral leaflets. we present our initial experience with the mitraclip system in 5 patients. in ...
